What is BCHIP – Bucks County Health Improvement Partnership
Bucks County Health Improvement Partnership was established over 25 years ago with support from our Bucks County Hospitals, The Bucks County Department of Health, and the Bucks County Medical Society in a shared vision of improving the health in Bucks County. The mission of BCHIP is to provide a leadership role in Bucks County through a collaborative approach to address gaps in health services and to better the health status of the Bucks County community. BCHIP believes that when all the major healthcare organizations in the county come together, we can make a difference.
Leadership/CEOs at each of Bucks County’s six hospitals support our programs:
• Advance Care Planning Assistance
o FREE counseling to help participants complete advance health care directives outlining their goals, values, and choices and document your wishes, a gift for your family and loved ones.
o Respecting Choices ® Facilitator training offered.
• Bucks County Immunization Coalition (BCIC)
o FREE coordination of COVID-19 vaccines for homebound, senior centers, & childcare centers.
o Advocating life-time vaccinations through various platforms specifically targeting our most vulnerable populations.
• BCARES (Bucks County Connect.Assess.Refer.Engage.Support)
o Warm handoff collaboration between hospital emergency departments and an assigned Certified Recovers Specialist (CRS) for individuals who have survived an opioid overdose.
o MAT and SBIRT offered for appropriate patients.
• Smoking Cessation Program
o FREE virtual and in-person group courses which run for five consecutive weeks (90 minutes each session) offering free nicotine replacement therapy for those that qualify.
• BCHIP Behavioral Health Committee
o Improved coordination of care for patients with mental health needs.
• FindHelp.org Project
o Promotion continues for this resource which connects free or
low-cost programs for but not limited to: Food, Housing, Health, Transit, Work, Education, Legal services, and goods.
• Long-Term Care Partnership
o To provide leadership for coordination and standardization of information and resources for the Bucks County Long-Term Care community.
• Bucks County Wellness Partnership
o Works to provide health education, health promotion, and screening activities in a cooperative, non-competitive manner. Coalition includes a wide variety of community partners.
• Access Matters
o Navigation Services for Title X Family Planning and Breast Cervical Screening Services.
BCHIP coordinates with our hospitals and community organizations to address the needs of clients and patients in Bucks County.
